Information for Referring Agencies

Any health and human service agency may refer clients for assessment for the Homelessness Prevention and Rapid Re-Housing Program (HPRP).

 

Referral Steps

  • Insure that your client meets the basic eligibility requirements set forth on the eligibility page of this site.

  • If the client meets those basic criteria, download the assessment form for your client and ask them to begin completing it.

  • Call the Homeless Assistance Coordinator at 479-280-2139 to set up an appointment for your client. This client meeting will occur at your location and will be scheduled during regular business hours.
